Parents/Guardians are required to fill out the Modified Sports Transportation Release form when taking your son/daughter home after an away game.  Please read below:

Instructions:

• Click here to complete the form.
• A new form must be completed each sports season. 
• List the dates you are taking your son/daughter home (you will only need to fill the form out once per season).
• If additional dates are needed, you will need to fill out the form again with those additional dates.

• Click on “forgot password” if needed to reset your password.
